The "Advance Directive Basics" page should provide you with most of the information you need about Advance Directives and contains links to forms you can use. FACILITATOR
TRAINING

This site provides an overview of how to train advance directive facilitators. To “facilitate” an action is to make it easier. Trained facilitators help a person go through, and understand, the advance directive form. For Advance Directive facilitator training materials and resources go HERE.
